Agreement for extended activity staff 19/11/2024 Agreement between extended school activity staff and the school Staff lead name(Required) First Last Activity nameWork / Mobile Phone(Required)Email(Required) Cost per session (£)(Required)If you provide another session or activity at a different cost, please complete a separate form.Consent(Required) I agree to the following principles:PURPOSE: To deliver skills/knowledge in a particular area to children at Extended School Activities MAIN PRINCIPLES: • All ESA staff will be subject to an enhanced DBS check; • The ESA staff member will uphold the mission statement of the school; • In respect of Heath and Safety and Child Protection Procedures, the ESA staff member will work within the guidelines set down in the school’s policies; • The ESA staff will liaise with senior management or Extended School Coordinator in respect of behaviour issues and expectations of children; • At the end of all extended school activities, the ESA staff will wait with the children until they are collected by parents, unless an agreement has been made previously to hand responsibility to a member of school staff. In any event, children must not be left unsupervised whilst waiting to be picked up; • I the event of ESA staff being sick or unable to take the activity then, where possible, advance notice should be given to the school office to facilitate communication with parents; • ESA staff will be shown and have access to the first aid room and any accidents will be noted in the accident book. Any injuries or incidents of concern must be reported to the adult collecting the child and to the Headteacher or Deputy Headteacher. If the child is going home alone, the peripatetic teacher/coach will phone the parental contact number to pass on the relevant information; • ESA staff must have a register of children’s names and record attendance for each session. For medical and emergency contact information, please liaise with a member of the office staff; • This agreement is underpinned by an understanding on behalf of all parties of the need to provide continuity in terms of personnel and the quality of teaching/coaching.Signature(Required)Date(Required) Day Month Year
